Responsibilities
- Conduct comprehensive psychosocial assessments of patients and families to identify needs, barriers, and supports.
- Develop and implement individualized discharge plans that support safe transitions from hospital to home.
- Collaborate with hospital teams, caregivers, and interdisciplinary colleagues to coordinate services, resources, and referrals.
- Facilitate access to community supports, social services, and durable medical equipment as part of the care plan.
- Document assessments, plan of care, and progress notes accurately in the patient record; ensure regulatory and agency compliance.
- Provide patient and family education, grief support when appropriate, and ongoing psychosocial follow-up throughout the home care episode.

Requirements
- Active Licensed Social Worker in New Jersey (LSW)
- Acute hospital experience required, specifically MedSurg and Emergency Department units
- Discharge planning experience is required
- Behavioral health experience will be rejected for this assignment
- Ability to perform in a home healthcare setting with communication and documentation responsibilities
